4、施工现场项目管理机构拟派人员满足**市园林绿化建设工程施工配备要求；标段最高限价400万元以下（含400万）的园林绿化工程项目，根据项目实际情况设置，最低应为4人（含）以上，且需包含园林项目负责人、安全员、质量员、材料员各1名。
5、拟派的项目负责人须具有**市园林绿化工程从业人员项目负责人电子信息卡。
6、提供依法缴纳税收和社会保障资金、近三年没有重大违法记录的书面声明。
7、未被“信用中国”、“中国政府采购网”列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单[以查询的报名开始之日止前三年内的记录为准]。
8、不存在投标截止日前一年内投标单位存在违反《**区关于加强建设工程招投标管理的若干意见》（浦建委建管[2020]26号）附件4“**区建设工程招投标领域市场主体信用管理操作办法”规定的失信违法行为，被行政处罚且在信用中国(**浦东)网站公示的（注：行政处罚以2023年1月1日（含）后为准，之前行政处罚不纳入否决）。标书代写
