简要规则描述：本次工作内容为****本部医院七层手术部医生办、值班、就餐、休息、会议室等区域，进行全部拆除，改造并扩建3间万级手术室，以及配套医护走廊、二次换鞋、缓冲、器械、污物通道、污物暂存等房间。八层设备层为手术室对应的空调机房，拆除和**机房一间。本次共计改造面积为410平方米。（具体工作内容，详见招标工作量清单）
Brief Specification Description: The work involves the seventh-floor surgical department of the main building of Shanghai Oriental Hospital, including areas for doctors' offices, on-duty staff, dining, rest, and meeting rooms. The scope includes complete demolition, renovation, and expansion to create three Class 10,000 operating rooms, along with supporting medical corridors, secondary shoe-changing rooms, buffer rooms, instrument rooms, contaminated material passages, temporary contaminated material storage rooms, and other facilities. The eighth-floor equipment level, corresponding to the operating rooms, will have one demolition and newly constructed air-conditioning machine room. The total renovation area is 410 square meters. (For detailed work content, refer to the bidding work quantity list)
合同履约期限：施工工期：90日历天，实际开工日期以采购人发出书面通知为准。
The Contract Period: Construction period: 90 calendar days, with the actual start date subject to the written notice issued by the purchaser.

二、申请人的资格要求
2. Qualification Requirements for Suppliers
(a)满足《****政府采购法》第二十二条规定；

3.2具备有效的安全生产许可证；
3.3项目负责人资质要求：拟派的项目负责人具有建筑工程专业二级及以上注册建造师执业资格，具备有效的安全生产考核合格证书；
3.4未被“信用中国”网站（www.****.cn）、中国政府采购网（www.****.cn）、国家企业信用信息公示系统（www.****.cn）等官方渠道列入失信被执行人、重大税收违法****政府采购严重违法失信名单且无其他不符合《****政府采购法》第二十二条的规定条件的情形。
3.5本项目不接受联合体投标。
(c)Specific qualification requirements for this program: 3.1 Construction Qualification Requirements: Must possess a Level 3 or above general contracting qualification for construction projects issued by the housing and urban-rural development authorities; 3.2 Must hold a valid work safety license; 3.3 Project Manager Qualification Requirements: The proposed project manager must have a Level 2 or above registered constructor qualification in the field of construction engineering and hold a valid work safety assessment certificate; 3.5 This project does not accept consortium bids.
