投标人需提供以下文件

*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标产品原产地在关境外的投标人必须是****公司。****公司，须由制造商总部提供其与制造商的关系证明文件的扫描件，关系证明文件必须签字盖章并提供有效联系人和联系方式，以便核实。
*（3）投标产品原产地在关境内的投标人必须是制造商（不接受代理商投标）。

投标方应提供 2021 年至今（以合同签订时间为准）应用于中国境内连续稳定运行 1 年及以上至少 20 台 DN100（4”）及以上且不锈钢阀体全通径、全夹套气动（气缸活塞式）开关球阀的成功使用业绩。业绩清单（包括：用户名称、供货阀门型号、数量、口径、压力等级、阀门数
据表、产地、应用装置名称、用户联系方式、投用时间），提供能够证明其销售情况的合同复印件和技术协议关键页复印件，可删除商务报价信息，并由投标方代表签字或盖章，投标方所提供的业绩必须真实、有效。

B.柱塞阀0720-2640zbzx0053/02 业绩要求：
投标方应提供 2021 年至今（以合同签订时间为准）应用于中国境内连续稳定运行 1 年及以上至少 20 台气动柱塞阀的成功使用业绩，其中至少包含 1 台 4”及以上且 CLASS 600 及以上的阀门。业绩清单（包括：用户名称、供货阀门型号、数量、口径、压力等级、产地、应用装置名称、阀门数据表、用户联系方式、投用时间），提供能够证明其销售情况的合同复印件和技术协议关键页复印件，可删除商务报价信息，并由投标方代表签字或盖章，投标方所提供的业绩必须真实、有效。
(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
(2) The bidder whose product origin is outside the customs must be the manufacturer or its subcompany;
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.****.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. (7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.

A. Performance Requirement: Pneumatic On/Off Ball Valve (Jacketed Valve)****
The Bidder shall provide the successful application performance of at least 20 DN100 (4) and above stainless steel full-bore, full-jacket pneumatic (cylinder piston type) switch ball valves that have been continuously and stably operated for one year or more in China since 2021 (subject to the time of contract signing). Performance list (including: user name, supplied valve model, quantity, caliber, pressure rating and valve number According to the table, place of origin, name of application device, contact information of user, time of putting into use), provide copies of the contract and key pages of the technical agreement that can prove the sales situation, delete the commercial quotation information, and sign or seal by the representative of the bidder. The Bidder shall provide the successful application performance of at least 20 pneumatic plunger valves that have been continuously and stably operated for one year or more in China since 2021 (subject to the time of contract signing), including at least one valve of 4 "and above and CLASS 600 and above.
