投标人需提供以下文件

*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标人必须是制造商；或者是制造商直接授权的代理商（进口产品开标前未进入中国关境内的，投标人必须****公司；进口产品开标前已进入中国关境内的，投标人必须****公司），代理商投标需具备以下条件： 标书代写
①必须提供制造商对本次投标的唯一正式授权书（授权书必须由制造商直接授权，不允许二级授权）；授权书中应包括“制造商对产品及售后服务质量负责”的相关内容和制造商联系人及联系方式，并确保制造商联系人能够协调处理项目执行过程中的重要事项。（对于此要求，制造商也可在授权书外另行出具书面承诺/说明），否则不接受代理投标；
②投标人依法登记注册时间距开标日满两年，且距开标日两年内有同类产品至少两个不同年份的销售业绩，需提供证明其销售业绩的合同或订单复印件或用户证明，并由投标人代表签字或盖章。 标书代写

*（6） 信誉要求
①投标人未被中国石油招标投标网(www.****.com)暂停或取消投标资格。（根据《****集团有限公司投标人失信行为管理办法》的规定，失信积分达到8分时，暂停失信行为人投标资格半年；失信积分达到9分时，暂停失信行为人投标资格一年；失信积分达到10分时，暂停失信行为人投标资格两年。暂停期满后恢复投标资格，有效期内的失信记分继续执行。失信积分超过10分或在暂停期间再次发生失信行为的，取消投标资格三年。不当异议的失信扣分详见《投标人失信扣分》。）

① The bidder should provide a letter with official authorized by Manufacturer for the bid；
② The bidder shall have sales performance for similar products in two different years within two years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(3)The bidder shall provide a copy of the valid certificate of registration such as the business license.
(4) The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)
(5) Other Qualification and Achievement requirements stipulated in Chapter VIII
(6) Reputation requirements

①The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.****.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) ②Bidders are not included in CNPC Suppliers, contractors, service providers blacklist (Suppliers, contractors, service providers blacklist show at www.****.com)
③If the bidder has a score of X for dishonesty, the bid evaluation price will be increased by X% on the basis of the bid price. The score of dishonesty is based on the inquiry results of the bid evaluation committee on the corresponding website (www.****.com). (Lowest bid evaluation method after evaluation) The above contents shall be subject to the inquiry results of the Bid Evaluation Committee on the corresponding website.
④The Bidder shall not be ordered to suspend production or business, temporarily withhold or revoke the permit, temporarily withhold or revoke the license, enter the liquidation procedure, or be declared bankrupt, or otherwise lose the ability to perform the contract. A letter of commitment signed by the legal representative or authorized representative and stamped with the official seal shall be provided.
